Medical Nutrition & Disease, 2nd ed.

Medical Nutrition & Disease, 2nd ed. G. Morrison, L. Hark. Malden, MA: Blackwell Science, 1999.

A potential companion to the Harriet Lane Handbook (M. Barone, ed), the second edition of Medical Nutrition & Disease provides the serious clinical nutrition student and health professional with refreshed and expanded chapters and case studies in four key parts. Those parts, written by an interdisciplinary team of clinical nutritionists, include Fundamentals of Nutrition Assessment, Nutrition Throughout the Life Cycle, Integrative Systems and Disease, and Fundamentals of Nutrition Support. The objectives of each area have been updated in an easy-to-read format that reflects contemporary nutrition issues and recent developments in clinical nutrition. On the other hand, the case studies presented reflect only slight modifications of those presented in the first edition. Answers to the case study questions are carefully worded, but need better referencing particularly for the uninformed reader.

"Fundamentals of Nutrition Assessment" was expanded to include recent developments in commercial weight loss diets and emerging evidence on the clinical significance of free radicals and antioxidants. The expanded tables on phytochemicals and disease prevention and potential benefits of some supplements are particularly relevant to current research interests.

"Nutrition Throughout the Life Cycle" was enhanced with the addition of several subsections, including issues and recommendations affecting racial and ethnic minorities and adolescents and a reorganization and expansion of issues affecting older adults. The tables on indications and types of infant formulas is markedly out-of-date (many cited products have not been in the US market for several years) and are poorly referenced.

"Integrative Systems and Disease" presents improved and expanded graphics, tables, terminology and general information on cardiovascular disease, gastrointestinal disease, endocrine disease, pulmonary disease and renal disease. For example, the relationship between cardiovascular disease and methyl metabolism is discussed and contemporary theories/terminology associated with diabetes is presented. On the other hand, presentations on GI and pulmonary disease management do not appear to reflect any recent developments in nutrition support. In addition, too little attention has been given to genetic factors, such as those associated with cystic fibrosis and endocrine disorders.

"Fundamentals of Nutrition Support" is significantly enhanced with two excellent sections on Enteral Nutrition and Parenteral Nutrition. The case studies on enteral feeding and esophageal cancer and indications for TPN in chronic pancreatitis are particularly appropriate and practical.

The chapter-review questions nicely reinforce the information to meet the stated objectives. These questions also support the continuing education hours for dietitians and nurses. Perhaps additional consideration may be warranted for those preparing for ACN and ASCN board certification.

The second edition of Medical Nutrition & Disease will prove to be a concise and valued desk reference for the practicing clinical nutritionist and university faculty member and a practical quick reference companion to the latest edition of Harriott Lane Handbook.
